For Manifestors, it can come from initiating without checking in, then dealing with consequences the body warned about in advance. For Generators and Manifesting Generators whose sacral is not the primary authority, the pattern looks like responding from the sacral while ignoring the spleen's deeper survival cues - saying yes to the response, but staying in a yes that is no longer safe.
